Born Sevenoaks, Kent, son of William and Annie Slade, of 1, Hartsland Road, Sevenoaks, Kent.
Previously served 3rd Battalion (Special Reserve), Royal West Kent Regiment.
Enlisted Tonbridge, Kent, age 25, height 5ft 11 inches, scar on back of right hand. Formerly carman.
Home 31st August 1914 to 31st May 1915; BEF 1st June 1915 to 26th March 1916.
