Enlisted Bromley, Kent, age 22 years 7 months, height 5ft 5 inches, weight 146lbs, scar on front of left knee.  Resided 18 Scotts Road, Bromley, Kent. Formerly bakers assistant.

(pyrexia of unknown origin); 12th November 1916, 11 Casualty Clearing Station, P.U.O. ; 29th November 1916, rejoined Battalion, "D" Company; 10th March 1917, appointed unpaid Lance Corporal; 9th April 1917, posted missing.

9th December 1917, date when Separation Allowance  of 16/- a week & Allotment of pay, of 3/6 a week, payable to wife will cease.

19th November 1917, pension of 18/9 a week awarded  to widow and one child.
