Enlisted 7th July 1916, Guildford, Surrey, age 39. Resided I, Herford Row, Upper Hale, Farnham, Surrey. Formerly labourer, employed by Farnham Gas & Electricity Works as a stoker for 8 years.

1st December 1916, discharged as "no longer physically fit for War Service" due to sickness. Silver War Badge issued

Disability total, tubercule of the lung, origin unknown, had pleurisy & pneumonia, has done practically no duty, under canvas on joining up. Not result of active service or aggravated by. Gratuity of £50.
