The name of Frederick Joseph Percy, contained in the accompanying certificate of birth I now declare to be my true name, and I make this solemn declaration conscientiously believing the same to be true, and by virtue of the provisions of an Act made and passed in the sixth year of the reign of King William the Fourth chap: 62, entitled The Statutory Declarations Act 1835
