Born St Chadds, Derby, enlisted 24th June 1916, age 35, height 5ft 7 inches, weight 126lbs, resided 28 Richmond Road, Derby. Formerly plasterer.
26th March 1917, enrolled at Derby, 3rd Battalion, Sherwood Foresters, 82559.
9th August 1918, Medical Report, Sutton, Surrey - Debility after Trench Fever, 4th October 1917 - very nervous & debilitated, suffers with neuralgia pains in legs, which are much wasted. Difficulty in walking, debilitated & somewhat emanciated.
31st August 1918, discharged as "no longer physically fit for War Service" due to sickness Silver War Badge issued.
18th June 1919, Derby, slight tremors, disablement 20%; 30th June 1920. Derby, slight tremors, disablement 20%; 4th July 1921, Derby, tremors of hands, pupils sluggish, disablement 20%; 5th July 1922, Derby, buzzing in head, trembling in feet, nervousness, heart & lungs sound, disablement 1 to 5%;
Gun shot wound, left leg.
