Enlisted 19th December 1915, Woolwich, age 52, height 5ft 3¾ inches, weight 126lbs, resided 74 Waldemar Avenue, Fulham. Formerly clerk.
Husband of Caroline Amelia Reynolds, (nee Hancock, spinster), married 7th August 1887, All Saints Church, Portsea, Hampshire.
Children - Florence Amelia Reynolds, born 2nd January 1893, Islington, N; Edith Beatrice Reynolds, born 7th August 1897, Islington, N.
19th December 1915, enlisted & posted Depot.
4th February 1917 to 28th March 1917, Fulham Military Hospital, pneumonia, feeling very weak on admission, requires dentures, to report to Dental Officer, Royal Herbert Hospital on discharge; 4th February 1917 to 11th July 1917 has improved since being discharged on 28th March 1917, he has lost around 1½ stone, digestion is much improved.
24th May 1917, Medical Report on an Invalid, Fulham Hospital - 3rd Royal West Kents attached Woolwich, ASC Records - disability, emphysema - origin date/place of disability, 26th January 1917, Woolwich ASC Records Office - Patient says he was in good health until about three years ago when he developed bronchial catarrh, was on one occasion only in bed for about a week. On 26th January 1917, he complained of cough and pains in chest and was sent home. Reported Fulham Military Hospital the following morning and on 4th February 1917 was admitted with pneumonia. Aggravated by drafts during reconstruction of Records Office. Patient suffers from sleeplessness and difficulty in breathing and shortness of breath on exertion. Patient has lost 2st 9lbs between January 25th and March 3rd 1917, but during the last two months has gained 1st 5lbs. There are signs of emphysema to a moderate extent. Herat normal. Blood pressure 130.
13th July 1917, discharged as "no longer physically fit for War Service" due to sickness Silver War Badge issued.
Pension, 5/6 a week for 26 weeks conditional, expires 15th January 1918.
24th July 1917, letter from man applying for an increase in pension. Report of Medical Board at Chelsea, 8th August 1917, prevents 50%. Increase current award to 13/9 a week from 245th July 1917.
13th November 1917, prevents 50%. Pension 13/9 a week for 78 weeks, expires 15th July 1919.
17th July 1919, pension 13/9 a week for 9 weeks pending Medical Report, expires 16th September 1919. Medical Report, 23rd August 1919, extremely weak, emaciated, confined to bed, attacks of vomiting, very feeble. Prevents 100% 6 months, 40/- & 10/- for wife.
12th September 1919, died.
